post: managing patients in elective and trauma wards, trauma calls and ward
referral, assessing patients in clinic, assisting in theatre and performing
minor surgical procedures. Hairmyres is a
pilot centre for Joint Replacement with Navigation System and I found it
interesting to learn the principles of this “computer assisted” surgical
technique. I attended
regular Radiology meeting. During this
position I have continued the research cycle of my ongoing projects and I
have carried out 2 audit projects. |

divided into elective and trauma weeks. My on-call commitments
at this stage were one in six. I was responsible for organising emergency
theatre lists, assisting in theatre, post-operative management, assessing
patients in Orthopaedic and Fracture clinics. I attended
weekly teaching sessions and Radiology meetings. During this
post I have carried out 3 research project and 3 audit projects. |

– 01/02/2005 (12 months) I was committed
in a one in eight rota, working 11 hours per day. In one year
time I assessed approximately four thousands patients including minor, major
and resuscitation cases. The “four hour target” was achieved in up to 99.8%
of cases. I attended
weekly teaching sessions, which included ALS and ATLS scenarios. I also had
the chance to attend regular Radiology teaching sessions. |

interesting and varied post while I was waiting to start my basic surgical
training. Duties of
this post included: medical assessment and management of people in custody,

extinct in suspicious deaths and appearing at Court as a Professional
Witness. I was committed for up
to 120 hours a week covering Dundee, Perth and Arbroath area. |

1 - BMI and early complications following knee
replacement Surgery. Patel A., Albrizio M. Level of involvement: research protocol, data
collection, data analysis, write up of the paper. A weak correlation between BMI and early complication following knee surgery has been found. There is a stronger correlation between surgeon and complications.
2 - BMI and early complications following hip
replacement Surgery. Patel A., Albrizio M. Level of involvement: research protocol, data
collection, data analysis, write up of the paper. A significant correlation between BMI and early
complication following hip surgery has been found. There is no correlation
between surgeon and complications. 3 - Correlation between percentage drop of
haemoglobin and length of hospital stay following joint replacement surgery. Patel
A., Albrizio M. Level of involvement: research protocol, data
collection, data analysis, write up of the paper. A statistically significant correlation between
percentage of haemoglobin drop and length of hospital stay has been found. |

Rate of positive CTPAs and predictive scores for the diagnosis of pulmonary
embolism. Level of
involvement: project lead. The rate
of CTPAs positive for Pulmonary Embolism has been evaluated in 100 patient
before and 100 patients after the implementation of the Wells score
as a predictive tool in Accident and Emergency. The implementation
has not led to a rise in the number of positive CTPAs. A pro-forma has been implemented to encourage junior doctors to make more use of the Well’s score. |

in prostate biopsy under Level of

Implementation of Digital Mammography: re-call rate and outcomes after 1 year
experience. Level of
involvement: project lead. A
significantly higher re-call rate (from 10% to 32%) has been found following
the implementation of Digital Mammography. This result has not been followed
by an increased number of detected cancers. |

Interventional radiology in the treatment of obstructive jaundice against
literature outcomes. Level of
involvement: project lead. Success rate
(post-operative bilirubin drop), 1 month and 1 year mortality rate have
been evaluated. 4 -
Antibiotic Prophylaxis in Orthopaedics and time of administration. Level of
involvement: project lead. The antibiotic
prophylaxis is delivered appropriately only in 7.5% of patients. A variation in
practice has been planned. Single dose of |

MRI and Arthroscopy of the Knee: Concordance of Findings at Local Level
Against Data Found in Literature. Level of
involvement: project lead. The
reliability of MRI scan in detecting knee pathologies and avoiding
unnecessary Arthroscopies has been successfully demonstrated. The concordance
of findings within the ranges found in literature. 6 -
Ten year follow up after total hip replacement. Radiological and clinical
review: telephonic interview as a cost-effective clinical alternative. Level of
involvement: project lead. The efficacy
of the 10 year follow-up has been disconfirmed. A variation
in practice has been suggested. Patient assessment via a telephonic interview
using a modified Harris hip score plus an A-P and L-L X-Ray. A re-audit has
also been planned. 7 - Influence of time of mobilization following joint replacement
on date of discharge. Level of involvement: data collection, statistical analysis, write up
of paper. The efficacy of an implemented “mobilisation on day 1 policy” has been
demonstrated with statistical significance. Presentation/publication: This work
has been presented at the Indian Orthopaedic Association International
Meeting – 8 - Compliance
of Operation Notes to Level of involvement: project lead. Audit Cycle: a new operation notes sheet with an aide-memoire has been
designed with the aim of matching the Royal College of Surgeons guidelines. |
